The quest for Gold begins in January for Team Memphis!

In order to participate in the JCC Maccabi Games, it is required that all athletes submit the following:
Signature Page
Official Games Medical Form
Passport size photo (If you do not have a passport size photo, the photo from your JCC Membership card can be used)
Copy of Birth Certificate or Passport for age verification
Copy of Health Insurance Card (cannot participate w/out insurance) & Prescription Card if separate.

IN ADDITION:

Each Athlete must complete on line registration at www.jccmacreg.org (select "Athlete" as your registration type).
