Camp Forms

Below are the forms required for your camper to attend Day Camp at the MJCC.  NEW Campers or Campers that did NOT attend 2012 Day Camp are required to complete ALL forms.  Returning Campers from 2012 Day Camp who submitted ALL required forms will only need to submit an updated copy of the Health Form unless there have been changes (i.e. contact information, allergies, immunizations, etc.)  Please complete and submit the required forms by one of the following methods:

 c. Drop off at the MJCC Member Services Desk
 d. Mail to MJCC | 6560 Poplar Avenue | Memphis TN 38138 | ATTN: Marcqualyn Pierce

Please Note: The Health Form does not require a doctor’s signature.  All forms must be completed, signed and returned by May 14, 2013.  If you have any questions, or need to verify what forms should be submitted for you returning Camper please contact mpierce@jccmemphis.org.
